Consumer Health Data Privacy Notice

Categories of consumer health data we may collect

When necessary for a consumer-requested insurance service, categories may include information about health conditions, diagnoses, medications, treatments, medical history, tobacco/nicotine use, height/weight, family medical history, or other health-related information relevant to insurance underwriting.

Centennial does not intentionally collect biometric identifiers, reproductive/sexual health data, or precise geolocation for marketing purposes.

Sources

Consumer health data may come directly from you, from documents you provide, or from insurers/authorized service providers where permitted and requested in connection with an insurance transaction.

Purposes

We may collect/use consumer health data to:

• respond to a request for insurance;
• prepare or support an insurance application;
• provide information to authorized insurers/underwriters as requested by you;
• service an insurance transaction;
• comply with law, prevent fraud, and maintain security; and
• perform activities otherwise permitted by applicable insurance or privacy law.

We do not use consumer health data for targeted advertising and do not sell consumer health data.

Sharing

Where covered by a consumer-health-data statute, Centennial will share consumer health data only:

• with your separate affirmative consent when required;
• as necessary to provide the insurance product or service you requested;
• with insurers, underwriters, licensed producers, and service providers involved in the requested transaction where permitted by law; or
• as required or authorized by law.

If a law requires consent to collect and a separate consent to share, Centennial will obtain those consents separately before the covered activity.

Your rights

Subject to applicable law and exceptions, you may request access, correction, deletion, withdrawal of consent, or a list of certain disclosures. You may appeal a denial where applicable.

Security

Consumer health data is handled using safeguards appropriate to its sensitivity. Do not submit health information through ordinary marketing forms, SMS, or unsecured email unless Centennial specifically directs you to an approved secure method.

Material changes

If Centennial materially changes the categories of consumer health data collected, purposes, or sharing practices in a way that requires new consent, Centennial will obtain the required consent before applying the new practice to covered data.

Nevada and other jurisdictions may impose additional content/consent requirements. Confirm exemptions and applicability with privacy/insurance counsel before collecting health information outside the insurer's approved workflow.
